The request form and the chat

The request form on the Request a Consultation page asks for your name, phone number, email and the office you prefer. The chat window, which opens from any page, collects what you type into it. What you send goes to HighLevel (also called LeadConnector), the system Evolve uses to receive and answer requests, so the team can contact you.

The chat asks whether you agree to receive text and email messages at the details you give. If you agree, you can reply STOP to a text to stop them.

Please don’t send health details through the form or the chat. Your provider goes over your health history with you at your consultation.

What else this website collects

The website has no accounts, and it does not run analytics, advertising or social media trackers.

Like any website, it is delivered by a hosting provider. When your browser requests a page, the provider’s servers receive technical information such as your IP address, your browser type and the page you asked for. The provider uses it to deliver the website and to protect it from attacks and automated abuse.

Cookies

The website’s own pages do not set cookies for analytics or advertising. The form and the chat are provided by HighLevel and may use cookies and a spam check (Cloudflare Turnstile) to work. The hosting provider may set a strictly necessary security cookie to tell people from automated traffic.

Links to other websites

Directions links open Google Maps, and some pages link to other websites. Those websites have their own privacy policies, which apply when you use them.

Your health information

Information you share with Evolve by phone or at an office is not collected through this website. Ask your office how it is handled.

This policy describes the website as it is on the date shown above, and it is updated when the website changes what it collects.
